Please list ALL the post-secondary institutions you have attended. Start with your current or most recent, include TST member colleges if applicable. You must arrange for official transcripts for every post-secondary institution you have attended. Transcripts MUST come directly from the issuing institution and be sent to the DMin Office. Photocopied or faxed transcripts are not accepted. If transcripts are in another language, an official English translation is required. If the name on your transcript differs from the legal name that you currently use please ensure that you include your former name in Section 1 above.
